A page of issues, and the open and closed counts for the same filters

Keyset paging: pass next_cursor back as cursor. q matches words in titles and bodies (SQLite FTS5): each word must appear, the last as a prefix, up to 10 words; comments are not searched. Results keep the chosen sort.
